The U.S. Chronic Disease Indicators (CDI) dataset brings together a standardized set of 115 measures that track key aspects of chronic disease and related risk factors across states and territories. Developed collaboratively by the CDC, the Council of State and Territorial Epidemiologists, and the National Association of Chronic Disease Directors, these indicators help jurisdictions consistently define, gather, and report chronic disease information that supports public health planning and practice.
